〔摘 要〕 目的：分析年轻乳腺癌患者临床病理特征及其预后。 方法：选取 2019 年 1 月至 2022 年 5 月南华大学附属第 二医院收治的 30 例年轻乳腺癌患者（20 ~ 40 岁）与同期就诊 30 例中老年乳腺癌患者（45 ~ 65 岁）作为研究对象，分别设 为观察组和对照组，比较两组患者的临床病理特征，并分析临床预后情况。 结果：观察组浸润性导管癌、组织分级 Ⅲ 级、 淋巴结转移、人表皮生长因子受体 –2（Her–2）阳性者占比均高于对照组；而浸润性小叶癌、组织分级 Ⅰ 级、雌激素受体 （ER）阳性、孕激素受体（PR）阳性者占比均低于对照组，差异均具有统计学意义（P ＜ 0.05）；两组患者的乳腺癌分子 分型情况比较，差异均无统计学意义（P ＞ 0.05）；经过 12 个月的随访，观察组患者病死率高于对照组，生存率低于对照组， 差异具有统计学意义（P ＜ 0.05）。 结论：年轻乳腺癌患者具有淋巴结转移情况及 Her–2 阳性高、组织分级高等临床病理特征， 与中老年乳腺癌患者疾病病理特征存在差异，对患者生存率的影响较大，预后不佳。
〔Abstract〕 Objective To analyze the clinicopathological features and prognosis of 30 young patients with breast cancer. Methods Thirty young breast cancer patients (20 ~ 40 years old) admitted to the Second Affiliated Hospital of University of South China from January 2019 to May 2022 and 30 middle-aged and elderly patients with breast cancer (45 ~ 65 years old) admitted to the Second Hospital of the University of South China during the same period were selected as the study objects, and were set as an observation group and a control group, respectively, to compare the clinicopathological characteristics of the two groups of patients, and the clinical prognosis was analyzed. Results The percentages of invasive ductal carcinoma, tissue grade Ⅲ, lymph node metastasis and human epidermal growthfactor receptor-2 (Her-2) positive in the observation group were higher than those in the control group, the differences were statistically significant (P < 0.05). The percentages of invasive lobular carcinoma, tissue grade Ⅰ, estrogen receptor (ER) positive and progesterone receptor (PR) positive of the observation group was lower than those of the control group, the differences were statistically significant (P < 0.05). There was no significant difference in the molecular classification of breast cancer between the two groups (P > 0.05). After 12 months of follow-up, the mortality rate of the observation group was higher than that of the control group, and the survival rate was lower than that of the control group, with statistical significance (P < 0.05). Conclusion Young patients with breast cancer have clinicopathological features of lymph node metastasis, high HER-2 positive and high tissue grade, which are different from middle-aged and elderly patients with breast cancer, which may have a greater impact on the survival rate of patients and a poor prognosis.
